Summer Session 2025: I Would Like to Register for the 2-Week Group Swimming Lessons

Please complete this form to register for Summer Session of group swim lessons being held on Mondays-Thursdays for 2 weeks at the Sports & Leisure Dome. Sessions running July 14-24, 2025 and August 18-28, 2025.

Upon completion of the form and submission of payment we will be in contact with you to confirm availability. Payment does not necessarily guarantee availability of lesson due to minimum class capacity levels. Programs not meeting registration requirements may be cancelled at any time. Programs may also be combined to meet the program enrolment minimums.

PAR-Q INFORMED CONSENT AGREEMENT & RELEASE WAIVER AND INDEMNITY
As a guest or casual user of this establishment, I acknowledge and accept the risk of injury or medical problems that could arise from my intended participation in the programs and services provided or from any other use of the facilities associated with my use. I also acknowledge that I have had the opportunity to undergo more detailed screening (the Par-Q form for example) for potential risks that I may knowingly have. I freely choose not to participate in such screening and hereby register with my full assumption of any such risks.

By signing, I declare that I have reviewed the Par-Q questionnaire and have no positive responses which would advise further medical clearance prior to my participation in any physical activity at the Sports and Leisure Dome. I also acknowledge having read, understood and agreed to the informed Consent Agreement. Each guest hereby releases the Nottawasaga Inn and its respective directors, officers, and employees of and from any claim whatsoever hereafter arising of any Guest suffering disease, deterioration of health, illness or aggravation of condition of ill health as a result of participation in the programs, acceptance of the advice or use of the facilities provided by the Sports and Leisure Dome. Any claim of personal injury sustained by any guest in or about the facilities including, without limitation, any claims for personal injuries resulting from or arising out of the negligence of the Nottawasaga Inn or it’s directors, officers or employees, or the negligence of any other person using the facilities, and acknowledges that he/she and any or all minors of the guest uses the facilities as his/her own risk.
